The following is a simple worksheet that will help with your submissions to the Restaurant Hunt Challenge.
Please send your answers and picture(s) in an email to blog AT foodtv DOT ca with Restaurant Hunt in the subject line.

- Restaurant name, street address and phone number
- Web address if applicable
- Cuisine (i.e. Italian, French, family diner, Chinese, pancake house, etc.)
YOUR COMMENTSHere are some questions that will help you to put together your feedback:
- Describe the food – is it a relatively faithful representation of the cuisine type, or more of an interpretation?
- What is your favourite dish there and why? If this is your first visit to the restaurant, describe the best discovery or favourite part of the meal.
- Describe the ambiance and/or setting of the restaurant, i.e. small and romantic, traditional décor, modern and casual, nestled in wine country, etc.
- Can you describe the crowd – what type of people frequent this spot? i.e. Families, college kids, young professionals, older couples, etc.
- Other: Is there anything particularly special about this place? A notable chef, long-time running, magnificent view, specialty dish or ingredient? Etc.
